Remote monitoring devices connect to the LinkMD platform. This data is shared with our Care Team. Patients also gain access to a digital health program that encourages the formation of healthy habits through high levels of engagement leading to lasting positive outcomes.
Our Care Team reviews device data and interacts with patients through calls and text messages. The Care Team encourages patients to engage with the health program, comply and adhere to the plan, ask questions related to their journey, and more so they receive the best care possible.
Our Care Team creates a summary report based on the remote data collected and their interactions with the patient. This summary is simple, yet conclusive, and in a format ready to send to insurance companies for billing.
As a provider, you receive a summary report from our Care Team. You review the data and notes, and then sign for approval. This report can then be sent to CMS as a billable event.
CMS review your signed report. Once approved, compensation is sent to you as the provider for meeting CPT code eligibility criteria.
